Patient & Care Partner Education: Free

Please utilize our extensive free education articles and webinars for step-by-step information on accessing medical care in BC and AB, and planning for medical needs & emergencies in advance.

is offered in Vancouver/surrounding cities, BC, and Edmonton, AB only. Remote consultation and support via phone/Zoom, & public speaking is offered to all of BC and AB.

Healthcare Navigation & Patient Advocacy

Our healthcare system is complex, fragmented, and under tremendous strain. Setting yourself, or a loved one, up in advance with experienced Patient Advocates ensures they are on call to be by your side, whether at a specialist’s appointment or for a sudden hospital admission. We understand the stress these situations can bring and are here to provide guidance, protective oversight, advocacy, and relief.


Services include, but not limited to:

  • Coaching patients + families as they navigate the healthcare system

  • Attending appointments as an expert witness + providing detailed summaries to patient’s care partners*

  • Hospital Discharge Planning to home or residential care

  • Reviewing and helping clarify complex diagnoses + treatment recommendations.


Fees:

  • Less than 5hrs: $175/hr

  • 5 hour package = $140/hr

  • Over 5 hours = $110/hr.

Premium Case Management

When someone is alone, aging, or very ill, a Healthcare Navigator can act as an overall case manager for their healthcare. Having a compassionate expert help them keep track of the many moving parts of their treatment, recovery or care can greatly reduce patient stress, and care-partner burden on busy or far away loved ones.


Services include, but not limited to:

  • Premium Case Management

  • Coordinating and overseeing all care needs + support teams

  • Setting up and attending all medical appointments*

  • Setting up and overseeing private-pay home and residential care

  • Providing frequent, detailed updates to loved ones

  • Coordinating, tracking, reminding patients of, and attending healthcare appointments


Fees:

  • Starting at $499/month, based on need

  • Overseeing hospital stays + discharge planning billed at separate rate

Advance Care Planning & Document Preparation

Plan ahead for a time you can’t speak for yourself due to serious injury, serious illness, or advanced cognitive decline.


Providing planning assistance & document preparation for:

  • Representation Agreements

  • Advance Directives that are in direct alignment with Medical Orders for Scope of Treatment (MOST), making your preferences for care seamless in a medical setting

  • Assistance with end-of-life requests, incl. Medical Assistance in Dying (MAiD)

Three sessions over Zoom are available to anyone in BC. The sessions include discussion and mentoring in determining values, beliefs and preferences for future healthcare.

Couples and adult children are encouraged to attend these sessions together at no extra cost.


Fees:

  • $400* - three to four sessions with accompanying homework

*based on approximately 3 hours of coaching and preparation time

End-of-Life Preparation, Navigation & Support

We are committed to respecting each individual's needs, wishes, and preferences when providing support to those who are planning ahead for, or are nearing, their final days. Services provided are non-denominational, and all faiths welcome.


Services include, but not limited to:

  • Preparing official Advance Directives

  • Preparing personalized comfort plans

  • Coordination of palliative care ie. in hospital, hospice, or in the home


Fees:

  • Less than 5hrs: $175/hr

  • 5 hour package = $140/hr

  • Over 5 hours = $110/hr.
