Premium Health Care Management

When a loved one is alone, aging, or very ill, a healthcare navigator will act as a care manager. We greatly reduce time and stress for care partners and families by being at the patient’s side, helping them manage all aspects of their care.


Services include, but not limited to:

  • Calls or visits to check in on a loved one daily or weekly, depending on need

  • Coordinating and overseeing all care support teams

  • Coordinating, tracking & reminding patient of all healthcare appointments

  • Attendance at medical appointments

  • Providing families with detailed updates


Fees:

  • Starting at $499/month, based on need

  • Ongoing rates established on a client-by-client basis

  • Hospital stays + discharge planning billed at separate rate

Advance Care-Planning & Document Preparation -

Plan ahead for a time you can’t speak for yourself due to serious injury, serious illness, or advanced cognitive decline.


Offering assistance in planning & document preparation for:

  • Representation Agreements

  • Advance Directives & Letter of Wishes for medical care

  • Establishing No CPR/MOST orders

  • Establishing Expected Death in the Home (EDitH) procedures

  • Establishing Medical Assistance in Dying (MAiD) requests

Sessions include discussion and mentoring in determining values, beliefs and preferences for future healthcare.

Fees:

  • $400 - three to four sessions with accompanying homework

Couples and adult children are encouraged to attend these sessions together at no extra cost.

Public Speaking

Connie Jorsvik is an experienced educator, empowering participants to take action in their Advance Care-Planning, and in understanding the healthcare system and their choices at end of life.
