FREE - please register. Advanced Care Planning is the foundation of being an informed and empowered patient and care partner. Join us in-person for a 2 hour overview of ACP documents, understanding resuscitation and levels of treatment, determining your own values and beliefs, and an introduction to writing a Letter of Wishes for Medical Care - and turning this into an Advance Directive, where appropriate.
