Our team supports people facing complex diagnoses, end-of-life planning, disability applications, Medical Assistance in Dying (MAiD) coordination and more. About the Opportunity
This is a self-employment opportunity for a Healthcare Navigator & Patient Advocate who wants to build and grow their own client-focused practice, while being affiliated with the excellence of the PatientPathways.ca team. You will operate as an independent professional, setting your own hours and building your own caseload, while benefiting from our established brand, mentorship, training, and referral network. The role involves supporting patients across British Columbia—particularly in the Vancouver area—in navigating healthcare systems and accessing appropriate supports and services.
The ideal candidate is empathetic, organized, experienced in healthcare or social services, and demonstrates excellent English verbal and written skills; a second language is an asset.
You will receive extensive training and ongoing mentorship.
Key Responsibilities
Provide one-on-one navigation support to patients, families, and caregivers in person, by phone, video call, and email.
Help clients understand care pathways, medical options, and eligibility for provincial services and benefits.
Facilitate access to MAiD assessments and care coordination in line with BC and federal legislation.
Coordinate referrals and communicate with healthcare providers, service agencies, and community organizations.
Assist clients with forms and applications related to disability supports, medical benefits, and end-of-life resources.
Maintain accurate, confidential documentation of client interactions in compliance with privacy legislation.
Participate in virtual and in-person outreach to educate the community about healthcare navigation resources.
Contribute to team collaboration, continuous learning, and quality improvement initiatives.
Qualifications
Required:
Post-secondary education in nursing, social work, healthcare administration, or a related field.
Minimum 3 years’ experience in patient navigation, advocacy, social services, or healthcare coordination.
Strong understanding of the BC healthcare system and provincial programs (e.g., MSP, Disability Assistance, Palliative benefits).
Excellent communication and interpersonal skills with a trauma-informed, person-centered approach.
Proficiency in digital platforms such as Google Workspace, Zoom, and EMR or CRM systems.
Ability to manage multiple cases efficiently while maintaining compassionate, individualized support.
Preferred:
Experience supporting MAiD processes, end-of-life care, or disability navigation.
Familiarity with Indigenous health services and equity-centred care.
Bilingualism (English/French or English/another language commonly spoken in BC).
Pay Structure
This is a self-employed position: you build and manage your own practice as a Healthcare Navigator & Patient Advocate.
Navigators retain 100% of their client earnings, setting their own rates and managing their own client caseload.
To maintain access to the Patient Pathways platform, brand, marketing, ongoing mentorship, case support, and training opportunities, contractors contribute a $500 monthly support and affiliation fee.
This monthly fee includes:
Mentorship and case consultation with senior navigators.
Continuing education, workshops, and professional development.
Marketing and promotion of your services through Patient Pathways’ website and referral network.
Access to shared tools, resources, and branding materials
