Advance Care Planning (ACP) is the umbrella of official documentation and courageous conversations that outline your healthcare + end of life wishes to loved ones and your healthcare team. You will leave this in-person workshop with Connie Jorsvik, Advance Care Planning Expert, knowing the “whys” and “hows” of Advance Care Planning, including how to identify and document your values, beliefs, and preferences for future healthcare and treatments if you are ever unable to speak for yourself due to accident, serious illness or dementia.

Additionally, you’ll gain the skills and confidence to have those important, sometimes difficult, conversations about your preferences with your loved ones and healthcare providers, and understand which legal and medical documents will ensure your wishes are known and respected.
