West Vancouver Seniors Activity Centre hosts a practical four-part workshop series presented by expert Connie Jorsvik of PatientPathways.ca .

In Part 2, you’ll learn the “whys” and “hows” of Advance Care Planning (ACP) in BC, Canada, including how to identify and document your values, beliefs, and preferences for future healthcare and treatments if you are ever unable to speak for yourself due to accident, serious illness or dementia.

Additionally, you’ll gain the skills and confidence to have those important, sometimes difficult, conversations about your preferences with your loved ones and healthcare providers, and learn what legal and medical documents will ensure your wishes are known and respected.
