An Advance Directive is the legally binding BC document that states the medical treatment you do and do not want to receive should you be seriously ill and incapable of not making decisions for yourself.

A Letter of Wishes is a non-legally binding document that gives your future decision makers and healthcare team guidance about the care you would and would not want to receive when you are seriously ill and incapable of making decisions. It allows them to make decisions in real-time based on circumstances.

Find out what document might be best for you, determine the best care level, and receive the appropriate template to write your own document.

You will leave this meeting with a beautiful healthcare-ready document outlining your preferences for resuscitation, and your values and beliefs.
